Late Policy

Our goal is to provide quality individualized medical care in a timely manner. "No-shows" and late arrivals inconvenience those individuals who need  access to medical care in a timely manner.  We know that delays can happen when you are trying to get to your appointment. However, we must try to keep the other patients and doctor on time. If you arrive 5 minutes past your scheduled time, we may have to reschedule your appointment. 
This policy enables us to better utilize available appointments for our patients in need of medical care. 
Repeated late arrivals may result in the patient and/or family being discharged from our practice.

Patients Without Insurance Coverage

We are happy to work with families that prefer to pay directly for services or do not have insurance. Payment is

due at the time of service.

Cancellation, Reschedule, and No-show Fees

We require notice of at least one (1) business day for all cancellations or reschedules. No-shows, same day cancellations, and same day reschedules will result in a fee of $30. This fee must be paid before the start of the next visit. Three missed (no-show) appointments may result in the family being advised to transfer care out of the practice.​

 

Forms

Immunization Records (3231), hearing and vision Forms (3300), and sports or camp forms brought to an exam are completed free of charge at well check appointments. Replacements and other forms that are requested for completion at a time other than an exam are $10 per form. **Due to the substantial amount time involved in completion, initial Katie Beckett applications are $75.

 

After Hours Nurse Line

Flourish Pediatrics provides an after-hours nurse line for any acute medical questions or concerns.
There is a $25 charge for nurse calls. You may file a claim with your insurance company if this is a covered service under your policy. Most insurance companies offer a free after-hours nurse advice line.

Refills

If you are requesting a refill be sent to the pharmacy, please allow up to three (3) business days for the order to be sent. Please be sure to confirm the pharmacy name, address, and phone number in your request.


ADHD Medication Refill Requests

Flourish Pediatrics only accepts online refill requests for ADHD medication. These will be processed within 72 hours Monday through Friday. Online prescription refills are not done on weekends and holidays. For those prescription requests received on Friday and weekend days, your prescription will be handled on the next business day. If you do not hear from us within that timeframe or should have any questions regarding your prescription refill or this online form, please contact us by phone at 470-500-6844. Remember, an appointment is required every 3 months to get refills for ADHD medication.
